How to Become a member – Northern Region

To get access to health care services with NHI:

You and all those living with you (including children or elderly) must have a valid Social Security Card. If you do not have a card you will not be able to receive the full NHI package of services!

To get a valid Social Security card for you and yourfamily members:

Minor:

  • Valid Belizean Passport, or
  • Child’s Birth Certificate with Parent Photo I.D
    • Photo ID such as: Passport, Voters ID, Social Security Card or Driver’s License (ensure that name on ID corresponds with the name on the birth certificate)
    • Name on birth certificate must match name on photo I.D.

Adult:

  • Birth Certificate with Valid Photo I.D.
    • Example: Driver’s License
    • Voter’s ID
    • Student’s ID with photo (14 years and older)
    • Document Personal de Identification (DPI), or Belizean Passport
  • Birth Certificate from Country of origin
  • Parent Photo ID
  • Documents showing legal status in Belize
    • Example: Permanent Residence
    • Temporary Work Permit
    • Dependent Permit
  • Original Certificate of Naturalization with photo
  • Document from country of origin e.g. DPI or
  • Belize Passport
  • Marriage Certificate
  • Deed Poll
  • Adoption Certificate
  • Pay $25.00
  • Police receipt/ report issued within the last six months

Golden citizen

  • Police receipt/report issued within the last six months
  • Card is free
  • Pay $25.00
  • J.P Declaration with $1.50 stamp

Golden citizen

  • J.P Declaration with $1.50 stamp
  • Card is free

I have my Card, how do I get care?

Once you have a valid Social Security card, and your address has been validated by the Corozal Social Security Branch Office, you and all your family members will have to register with an NHI Primary Care Provider (PCP) clinic.

The requirements for proof of address for the validation of registered head of household and all family members living at the same address is:

  1. Utility Bill in name of head of household or J.P. Declaration of place of residence with $1.50 stamp
  2. J.P. Declaration of all family members living at same residence with $1.50 stamp must include each family member by name, their social security number and date of birth

There are two available clinics in Zone 1 of the Corozal District. These are Presbyterian Medical Clinic in Patchakan and the Corozal Primary Care Provider in Corozal Town. You need to register with the one nearest to where you live.

To register with the clinic, simply take you and your family’s individual Social Security cards to the clinic you are assigned to. They will help you register with them.

Once this is complete, you and your family members can all access health care when needed.

Once you have registered with a clinic, you must go to the same clinic when you need medical attention.
