Transferring to another NHI Clinic
Based on where you live in the Southern Region there is an assigned clinic you need to register with. However, if you then move to a different district in the southern region you may need to transfer to that designated clinic.
If you wish to change your Primary Care Provider Clinic, remember this:
- You must request a change of address at the local Social Security office using the available forms.
- Once a transfer is carried out by SSB, you will be given an NHI slip to present to the new NHI clinic who will then register you and your family.
- Once you have received this authorization, you can access care from your new provider.
